Why authorship disputes keep happening
Authorship disputes are one of the most common-and most avoidable-sources of friction in multi-author manuscripts, particularly ones involving medical affairs teams, clinical investigators, and external key opinion leaders. The disputes rarely come from bad faith. They come from a shared but incorrect assumption: that meaningful involvement in a study automatically earns a byline.
The International Committee of Medical Journal Editors (ICMJE) defines authorship through four specific criteria, and all four must be met for someone to be listed as an author. Understanding them-and applying them early-prevents most of the disputes that otherwise surface at submission, when they’re hardest to resolve.

Authorship is not a single bar to clear. It’s four separate ones, and a contributor needs to clear every one of them.

Meeting only one or two of these criteria qualifies a contributor for acknowledgment-not authorship.
Substantial contribution
Either to the conception and design of the work, or to the acquisition, analysis, or interpretation of the data. This is usually the criterion people assume is sufficient on its own. It isn’t.
Drafting or critical revision
Actually writing the manuscript, or critically revising it for important intellectual content. Light editorial comments or formatting suggestions don’t meet this bar.
Final approval
Reviewing and formally approving the final version submitted for publication-not a general awareness that the paper exists.
Accountability
Agreeing to be accountable for the accuracy and integrity of the entire work, not just the section they personally contributed to.
All four criteria must be met-not just one or two.
What doesn’t qualify someone as an author
This is where most disputes actually originate. A number of genuinely valuable contributions to a study don’t, on their own, meet the bar for authorship-and ICMJE is explicit that these should be acknowledged, not credited as authorship.
